Estimated Credentialing Time:
- 6-8 weeks maximum
- Skillset/capabilities/requested procedures:
- Proficient in peripheral arterial disease management and treatment
- Proficient in diagnostic and percutaneous coronary intervention
- Proficient in general cardiology clinic setting

- Group retains the rights of first refusal at fair market value of OBL if Physician decides to leave the practice or is terminated after year 3.

- Requirement of collections of 1 million in gross revenue in year 3 to be considered for a 3% ownership in Clinic at commencement of year 3, Group retains the rights of first refusal at fair market value of Clinic if Physician decides to leave the practice or is terminated after year 3.
