* Demonstrated competency as a physician by the completion of an appropriate residency program; board certified or board eligible.
* Must be able to obtain medical staff privileges.

* Licensed or eligible for State of Iowa physician license; Iowa Controlled Substance Certificate, and Federal DEA Certificate
