Responsibilities:
- Drive high-quality, safe, and cost-effective patient care in post-acute care communities
- Collaborate with interdisciplinary teams and follow CMS guidelines
- Work closely with hospice, palliative care, and home care agencies for safe patient transitions
- Ensure timely documentation and billing through our EMR (Gehrimed) meeting Medicare guidelines
- Candidates will be picking up a weekend shift every other weekend as a part of supporting our full-time advanced practice clinicians
- On shift, typically rounding between 5-10 patients per day
- Experience in internal medicine, family medicine, or geriatrics preferred
- Previous experience in post-acute care, traditional medicine, outpatient, urgent care, or hospitalist settings is a plus
