Responsibilities:
  • Provide primary care services in post-acute care facilities, including skilled nursing and assisted living
  • Drive high-quality, safe, and cost-effective patient care
  • Collaborate with interdisciplinary teams and follow CMS guidelines
  • Work closely with hospice, palliative care, and home care agencies for safe patient transitions
  • Provide oversight, guidance, and mentorship to clinicians within facilities
Our compensation is a productivity-based model (fee for service) that offers you control to maximize your income potential. Expected compensation is estimated salary range of $206,000 to $277,000 annually with no cap productivity income potential. This full-time role is benefit eligible.

Position Details:
  • Sign-on bonus: $10,000
  • Schedule: Monday through Friday
  • Patient volume: 20 patients per day
Qualifications:
  • Background in providing primary care in internal medicine, family medicine, emergency medicine, urgent care, PM&R, or geriatrics
  • Ability to work flexible schedules
