-Experience required.

**Job Description**
-Provider must have minimum of 2 years in practice in outpatient primary care, rural health clinic experience preferred.
-Able to work autonomously.
-Must be Board Certified.
-Office location is in Colebrook NH.
-Direct patient care time will be split between acute care and established patient panel.
-Replacement for provider that is retiring.

**Primary Care:**
-Pediatric through Geriatric.
-High elderly population.
-Experienced in Chronic Disease Management.
-16-22 patients per day.
-36 patient hours, 4hrs of admin time - NO CALL.
-EMR is Meditech Expanse, Dragon dictation.
-Supported by an RN Team leader and medical assistant.
