Nurse Practitioner Job In Findlay, Ohio
Palliative Care Nurse Practitioner
We are seeking a dedicated Palliative Care Nurse
Practitioner to join our team in Findlay, OH. This position focuses on symptom
management, chronic disease management, goals of care, and advanced care
planning. You will provide 100% inpatient care, rounding on inpatient hospice
patients Monday through Friday. The schedule consists of a 5-day workweek, 8:00
am to 5:00 pm, with a rotating on-call requirement. You will manage an average
of six patients per day, and the hospitalists will handle admissions. The facility
has an open ICU.
Qualifications:

Board Certified Nurse Practitioner (NP)
·
Geriatric Certification or willingness to
obtain within one year
·
Active Ohio license or willingness to obtain
·
Minimum of three years of experience as an NP
in an acute care setting, or with Palliative Care and Hospice
