Nurse Practitioner - Outpatient

We are seeking a dedicated and compassionate full time Nurse Practitioner to join our outpatient clinic in Lower Manhattan, NY. The ideal candidate will provide high-quality healthcare services to primarily Chinese-speaking patients, ensuring they receive the best possible care in their preferred language. The role involves coordinating and supporting participants' functional, clinical, and psychosocial needs, prioritizing independence and wellness. Responsibilities include serving as a liaison between participants, community physicians, and plan staff to develop and implement care plans, including person-centered goal development, communicating changes in participant condition, and facilitating primary care preventative services.

Compensation and Benefits: $165,000 - $175,000 plus a full benefits package.

Requirements:

• Master's degree in Nursing from an accredited institution.
• Current licensure as a Nurse Practitioner in the state of New York.
• Certification as a Family Nurse Practitioner.
• Minimum of 2 years of experience in family medicine or primary care settings.
• Strong clinical skills with the ability to manage a wide range of health conditions.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a collaborative team.
• Proficiency in EHR systems.
• Bilingual in Chinese preferred but not required.
