Job Details
- 17 patients per day
- Full-time position: 5 days per week, Monday through Friday
- Compensation and benefits: our compensation is fee for service (FFS); expected compensation is estimated salary range of $111,000 to $161,900 annually with no cap on productivity income potential - this full-time role is benefit eligible
Key Responsibilities
- Perform primary care in a post-acute setting for short term and long-term patients
- Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care
- Order and interpret lab results
- Prescribe and monitor medications
- Consult with physicians and coordinate patient care with other healthcare practitioners
- Educate patients and their caregivers on coping skills to handle chronic and age-related conditions
- Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery
- Promote healthy lifestyle changes
Qualifications
- A minimum of 1 year experience as a nurse practitioner in a primary care setting required
- Post-acute care experience a plus, but not required
- Active and unrestricted New York NP license required
- Active and unrestricted DEA with prescriptive authority required
