Nurse Practitioner - Family Practice

- Specialty: FNP or NP with Peds experience (Ages 3-21)
- Start Date: ASAP, As soon as credentialed
- Schedule Details: M-F 8a-5p
- Contract Length: 3 months ongoing
- Schedule: Provider will be able to schedule out available days a month of more in advance, but looking for 4-5 days/week
- Work Details: In-home Well Child Visits
- Patients per day: Goal of 4-6 Assessments per day,
- Department Description: Provider will ideally receive scheduled visits 1 week in advance. Homes will be scheduled as close together as possible for any given day.
- EMR: Athena
- Skills Required: At least 1 year of experience conducting child wellness visits Travel Logistics: All Inclusive BR Requirements:
- License Required: AZ NP. NEEDS to be AZ Medicaid certified (Health Choice preferred)
