Role:
- Provide primary care in a skilled nursing facilities in Creswell, Oregon, 1 day a week
- Provide primary care treating a diverse patient population, manage chronic conditions, and collaborate with a multidisciplinary team
- Build long-term relationships with patients for improved care continuity
Responsibilities:
- Deliver primary care for short-term and long-term patients
- Manage chronic diseases, diagnose conditions, and create care plans
- Order and interpret lab results, prescribe medications, and consult with other healthcare providers
- Educate patients and their caregivers, and promote healthy lifestyle changes
Requirements:
- Must have a minimum of 6 months primary care experience
- Post-acute care experience is a plus
- Active and unrestricted Oregon NP license and DEA, or application in progress
