Pharmacist Clinician Anticoagulation/Full-Time

POSITION SUMMARY: Primary functions are assessing, planning, delivering, coordinating, teaching and providing patient disease management (anticoagulation, hepatitis C, lipid, smoking cessation, vaccinations, etc.) care/treatment and department services. On occasions assumes charge responsibility. Serves as preceptor for new hires, students and resource person for other staff.
Requirements
MINIMUM QUALIFICATIONS:
EDUCATION: Graduate of an ACPE accredited School of Pharmacy. Bachelors of Science in Pharmacy or Doctor of Pharmacy required.
CERTIFICATION/LICENSES: Licensed Registered Pharmacist (RPh) in the State of New Mexico.
Licensed Pharmacist Clinician (PhC) by the New Mexico Board of Pharmacy
Registered as a Pharmacist Clinician with the Board of Medicine to exercise prescriptive authority in accordance with current/active guidelines or protocols as defined by the Pharmacist Prescriptive Authority Act, Section 61-11B-1 through 61-11B-3 NMSA and 16.19.4.17 NMAC-Rn
Certification in Basic Life Support for Healthcare Providers (BLS)
Advanced training/certification in anticoagulation and/or disease management
SKILLS: Gathering and analyzing physiological, psycho-social and socio-economic data; accurately evaluating patient condition in person or via phone; recognizing changes in patient status and ordering appropriate diagnostic tests and therapies; educating, listening and communicating with patients and families, physicians and other staff.
Ability to read, write, and speak English fluently required; fluency in Spanish preferred. Strong verbal, written and interpersonal communication skills. Ability to work collaboratively with others. Clinical pharmacy skills including physical assessment, planning, delivering and evaluation of care required. Ability to work at a rapid pace under stressful conditions.
EXPERIENCE: Direct patient care experience as Pharmacist Clinician with prescriptive authority. Experience current in disease management
NATURE OF SUPERVISION:
-Responsible to: Anticoagulation Management Services Coordinator and Pharmacy Director
ENVIRONMENT: May be exposed to physical injury, toxic substances, infectious diseases and bloodborne pathogens. Requires knowledge of safety standards to limit exposure to infectious diseases.

PHYSICAL REQUIREMENTS: Bend, stoop, lift up to 20 lbs., reach above head level and below knee level. Move around clinic.
