Summary:

The Manager Clinical Pharmacy is responsible for the coordination and development of clinical pharmacy services in coordination with the Director of Pharmacy and the Pharmacy Operations Manager. Responsibilities include but not limited to: ensuring pharmacists provide optimal clinical services daily, acting as departmental liaison in all clinical matters, student preceptorship, intra- and interdepartmental clinical education programs, staff development and clinical competency, oversight and evaluation of the clinical pharmacy staff, and chairing meetings. The Manager Clinical Pharmacy facilitates the development, implementation and maintenance of systems that promote desirable patient outcomes through the use of safe, appropriate and cost-effective medication therapies.

Recommends and/or establishes interdisciplinary teams supporting pharmaceutical care and patient care services. Nurtures collaborative pharmacist-physician and pharmacist-nursing relationships to provide safe, effective, and efficient patient care while ensuring pharmacists are empowered to serve as patient representatives/advocates.

Responsibilities:

• Meets expectations of the applicable OneCHRISTUS Competencies: Leader of Self, Leader of Others, or Leader of Leaders.
• Proficient in living the Mission and Core Values and coach/mentor Ministry pharmacy leaders to optimize performance of pharmacy in each ministry.
• Ensures pharmacists, technicians, interns, and residents receive ongoing training, competency assessment and performance improvement activities to improve patient outcomes.
• Collaborates with providers to maintain an effective hospital formulary based on safety, efficacy, and cost; as well as the development of treatment guidelines / protocols that improve clinical care.
• Oversees and contributes to the Pharmacy & Therapeutics Committee. Maintains expertise in assigned clinical pharmacy practice and with current clinical guidelines and literature, clinical assessment and professional communication skills that is appropriate for oversight of clinical pharmacy service lines.
• Coordinate and oversee strategies for antimicrobial stewardship. Collaborate with interdisciplinary teams or stakeholders such as physicians, nurses, and other allied health professionals to ensure best practices, guidelines, policies, and care processes to ensure positive outcomes, decreased morbidity and mortality, and management of drug costs. Produce and review yearly antibiogram. Educate physicians, nursing, staff and patients.
• Coordinate and support strategies for opioid stewardship. Collaborate with interdisciplinary teams or stakeholders such as physicians, nurses, and other allied health professionals to develop and implement best practices, guidelines, policies, and care processes related to opioid prescribing and pain management. Educate physicians, nursing, staff, and patients on pain management and pain expectations, as well as safe opioid prescribing, storage, and disposal. Ensure safe patient care through the prevention of adverse drug events and improving safe and effective use of opioids while also adequately controlling pain.
• Serve as the pharmacy contact for evidence-based order set review. Facilitate incorporation of drug therapy best practices into the order sets utilizing clinical pharmacy expertise.
• Coordinate and lead strategies to manage industry drug shortages, including development of policies, guidance documents, and tools. Assist with research and coordination of supply chain alternatives (such as development of therapeutic interchanges, conservation strategies, and supply source or contracting alternatives).
• Consult and collaborate with stakeholders such as pharmacists, physicians, nursing, clinical educators, legal counsel, and ethics leaders, as appropriate.
• Assume responsibility for ongoing benchmarking related to clinical pharmacy services, including productivity assessment and impact of clinical services on drug utilization, costs, and medication safety goals.
• Provides appropriate drug information services, in-service education, and communication to keep pharmacy, physician, and nursing staff aware of important changes in pharmacy policy, procedure, or formulary status. Develop pharmacy education intiatives and development and/or improvement of the pharmacy clinical service model.
• Develop, recruit, train, and mentor Clinical Pharmacist, residents, and students.
• Serves as a preceptor for pharmacy students and/or residents from local colleges of Pharmacy, pharmacy residents (if applicable), and/or other healthcare professionals in training.
• Collaborates with the Director of Pharmacy and/or Pharmacy Operations Manager to review and maintain department policies and procedures for pharmacy services; Assists in development and implementation of accrediting agency medication management standards. Facilitate and provide leadership in the maintenance and development of clinical pharmacy initiatives, policies, and protocols.
• Coordinates Performance Improvement/medication safety initiatives (including Medication Use Evaluation (MUE); medication incident reports, and adverse drug reactions (ADRs) for reporting to appropriate committees. Participates in departmental and interdisciplinary committees to support the organization’s efforts for performance improvement in the areas of patient safety, therapeutic outcomes, and cost savings.
• Enhances personal professional growth and development by accessing educational programs, job related literature, in-service meetings, and workshops/seminars.
• Responsible for remaining up-to-date on all Federal, State and local laws, accreditation standards or regulatory agency requirements, which apply to the assigned area of responsibility and ensures compliance with all such laws and regulations.
• Performs other related duties as assigned by manager including but not limited to:
• Clinical Intervention activities (IV to PO, Renal Dosing, Pharmacokinetic dosing support, Anticoagulation)
• Baseline departmental standards for reporting interventions for clinical productivity as defined by departmental goals.
• Attends interdisciplinary rounds.
• Contributes to the advancement of the pharmacy department and medication management through-
• Formal in-services of pharmacy, nursing, and medical staff
• Participation in scholarly activities as assigned (i.e. peer-reviewed poster presentation, publication, platform presentation, provision of continuing education program)
• Participation in professional activities, organizations, or journal clubs
• Faculty appointment as assigned by management
• Competency development, recruiting activities, and staff development projects

Requirements:

  • Doctor of Pharmacy or BS Pharmacy degree required plus o PGY1 Pharmacy Residency with 5 years hospital experience OR PGY2 Pharmacy Residency/Fellowship with 2 years hospital experience OR Fellowship OR significant clinical experience may be considered in lieu of residency or fellowship. 
  • 5 years of health-system experience preferred. 
  • 3 – 5 years in a clinical pharmacist position required. 
  • Contributor to the functions of the Pharmacy & Therapeutics Committee and related processes. 
  • Contributor to an antimicrobial stewardship committee as a pharmacy leader (preferred) or member. 
  • Must be proficient with Microsoft Suite. 
  • Must have knowledge of hospital EHR and associated pharmacy platforms. 
  • Pharmacy State licensure required at or within 60 days of start date. 
  • Board Certification (e.g. BCPS, BCCCP, BCOP, BCIDP) required within 1 year of hire. 
  • Pharmacy preceptor licensure within 90 days of hire is required. 
  • BLS or ACLS required within 180 days of the start date. 
  • PALS (pediatric practice) required within 180 days of the start date. 
  • ASHP Residency Preceptor status required within 180 days of hire for Ministries with Residency programs.
