Family Medicine Nurse Practitioner
We
are seeking an experienced Family Medicine Nurse Practitioner to join our
dedicated team in providing comprehensive primary care services across four
Phoenix-area locations. This full-time position focuses on delivering
high-quality, patient-centered care to a diverse population in Midtown,
Northwest Phoenix, Guadalupe, and Mesa. The ideal candidate is a compassionate
professional with a solid background in family practice and a passion for
serving the community.

Requirements:
- Graduate
of an accredited Nurse Practitioner Program and certified by the American
Nurses Credentialing Center or the American Academy of Nurse Practitioners.

Must
have a permanent and unrestricted license to practice as a nurse practitioner
in Arizona.

Must
have current board certification in family medicine and maintain certification.

Must
have current DEA license.

At
least 3 years of experience in a family practice or comparable setting is
required. Experience in a community health center setting is preferred.

Current
ACLS certification.

Must
possess a valid and unrestricted Arizona driver’s license (within 30 days of
hire if applicant is from out-of-state) with no DUI/DWI or reckless driving
convictions in the last five years. No more than two at-fault accidents in the
last three years. Must maintain a valid Arizona driver’s license during
employment. Must be insurable under our liability auto policy.

Must
pass a criminal background check and obtain a Class I Fingerprint Clearance
Card within the initial ninety (90) calendar days of employment and retain a
valid card while employed.

Must
pass substance abuse testing upon employment and submit to random testing
during employment.

Must
provide an updated immunization (IZ) record at the time of employment.

Must
have a current (within 12 months) Tuberculosis (TB) skin report upon employment
and provide an annual update.

Must
obtain a First Aid/CPR Card within (90) calendar days of employment and
maintain a valid CPR Card during employment.
