Community Physician Group is looking for a dedicated and experienced Nurse Practitioner or Physician Assistant to join our Post Acute Care team. With this addition, the team will consist of one physician, two nurse practitioners, and an RN to support practice coordination. This role involves providing compassionate care for patients in sub-acute care for short-term rehabilitation as well as long-term care at four skilled nursing facilities within the local communities of Missoula and Stevensville, MT.Schedule:
- Clinic Hours: Monday – Friday, 8 AM - 5 PM
- Setting: Local skilled nursing facilities for both sub-acute short-term rehabilitation and long-term care
Key Responsibilities:
- At least 2 years of experience in adult primary care is required and experience in geriatrics and/or palliative care is preferred.
- Visits include routine follow-up, acute visits, discharge visits as well as providing palliative and end of life care within the SNF setting.
- Duties would include assessments, ordering and follow-up of diagnostics tests, prescribing medications, and collaboration with specialists. Additionally, given this vulnerable population, including families / health care representatives in updates on health status and medical decision making is also an important aspect of this position.
- Call is shared across the entire CPG Primary Group and is phone call only.
- Ability to work independently required
- Flexibility/adaptability is required.
Requirements:
- Minimum of 2 years of experience in adult primary care.
- Experience in geriatrics and/or palliative care is preferred.
- Strong ability to work independently.
- Flexibility and adaptability to meet the needs of patients and facilities.
