·Arranges for the post-discharge needs of patients.
·Collaborates with providers, nurses, patients, families, and other care providers in the development and implementation of Quality programs and initiatives and patient care plans, including discharge planning, across the continuum of care in support of the GCRMC Mission.
·Communicates with providers regarding patient status and assist him/her in ensuring proper placement of patients.
·Demonstrates familiarity with the various levels of care and community resources available for each patient and the providers of these different aspects of care.
·Utilizes innovative methods to facilitate development and implementation of discharge plans.
·Attends Community Health Workers Initiative and New Mexico Department of Health meetings as directed.
·Perform advocacy activities to support vaccine equity.
·Provides COVID-19 related outreach and education to underserved and racial/ethnic communities to promote vaccine health literacy.
·Conducts outreach activities which may include community-based activities such as door-to-door outreach, attendance at community events, and assistance with the planning and coordination of vaccination clinics.
·Promotes the importance of self-care and other preventive health measures.

Prepares and turns in monthly activity reports, compiles and formats data necessary for departmental reports.
·Demonstrate knowledge in a medical office/medical terminology.
·Facilitate physician and patient communication.
·Answer incoming calls in a professional and courteous manner with minimum rings.
·Route phone calls and handle all enquires efficiently and with courtesy.
·Welcome patients and visitors in a polite and courteous manner.
·File and fax documents appropriately.
·Contribute to team effort by accomplishing related tasks as needed.
Participates in quality improvement, continuing education, and evaluation activities and other duties as deemed necessary.
