**Locums Geri Nurse Practitioner - Mishawaka, IN** **Coverage Need** - Reason for Coverage: Provider on leave - Start Date: ASAP - End Date: 6 months - Hours: Full-time (Part-time will be considered) - On Call: One week on, one week off - Call Coverage: Weekdays from 4:30 PM to 8:00 AM, weekends for 24 hours **Position Details** - Certifications Required: - AANP or ANCC Board Certified - BLS - CDS - DEA - Provider Responsibilities: - Perform assessments in collaboration with PACE physicians - Develop and implement care plans - Provide primary medical management - Educate participants and caregivers - Participate in the Interdisciplinary Team (IDT) - Perform rounds in contracted nursing homes - Provide oversight of PACE organization clinics - Document medical conditions and changes in participant records - Practice Population: Elderly, frail population **Submissions Requirements** - Active State License: Indiana - Board Certification: AANP or ANCC - Experience with Elderly/Frail Population: Minimum one year preferred - Clean Background: Required - IMLC Accepted: No
