**Locums Nurse Practitioner - Mishawaka, IN** Located: Midway between Chicago IL & Fort Wayne IN **Assignment Dates** - 6-month assignment to start. Potential for Ongoing. Rate: Negotiable (tell us your rate) Schedule: Negotiable (tell us your preferred schedule) **Coverage Need** - Full-time preferred (Part-time considered) - Scheduled clinic hours with on-call responsibilities: - On-call one week on, one week off - Weekday on-call: 4:30 PM to 8:00 AM - Weekend on-call: 24-hour coverage - Phone-only on-call (no travel required) **Position Details** - Location: Mishawaka, IN - EMR: TruChart (transitioning to Epic in March) - Geriatric experience required - Provides care within a PACE (Program of All-Inclusive Care for the Elderly) model: - Assessments, care planning, treatment, participant education, and team collaboration - Oversight of the clinic and contracted nursing home rounds - Interdisciplinary Team (IDT) participation - Documentation of participant care and medical condition updates **Submissions Requirements** - Active IN license - Clean History - Board Certified (ANCC or AANP) - Up-to-date CV with explanations for gaps >30 days - Proof of active IN license and board certification - Supporting certifications BLS, CDS, DEA **Skill Requirements** - Geriatric care experience - Experience with elderly or frail populations (preferred) - Independent judgment and decision-making abilities - Strong communication and organizational skills - Proficiency in Microsoft Office and EMR systems
