Nurse Practitioner in Chesterton, IN

Are you a nurse practitioner (NP) or a physician assistant (PA) with a passion for adult patient care and enjoy solving complex medical problems within an interdisciplinary medical team? This part-time primary care role will include patient care rounding in a post-acute care setting in a skilled nursing facility (SNF) located in the Chesterton, Indiana area.

About the Role:
As a weekend provider in a skilled nursing facility environment, you will be an integral part of our care team, providing comprehensive primary care services to our acute patients. Your responsibilities will include:

• Conducting thorough patient assessments and developing individualized care plans
• Prescribing medications and treatments
• Collaborating with physicians and other healthcare professionals
• Providing education and support to patients and their families

Qualifications:

• NP/PA license
• Active and unrestricted medical license in Indiana
• Strong clinical assessment and diagnostic skills
• Excellent communication and interpersonal abilities
• Experience in post-acute care a plus

Expected compensation is estimated salary range of $27,450 to $50,450 annually with no cap on productivity income potential.
