Job Description

About the Role:
You will play a vital role in delivering compassionate and effective care to our patients in post-acute care communities. Your responsibilities include:
  • Providing comprehensive primary care services to a manageable patient panel of 15-20 patients per day
  • Collaborating with interdisciplinary teams to optimize patient outcomes
  • Ensuring smooth transitions of care with hospice, palliative care, and home care agencies
  • Maintaining accurate and timely documentation in our EMR system (Gehrimed)

Qualifications:
  • Graduate degree as a nurse practitioner or physician assistant
  • Active and unrestricted NP or PA license in North Carolina 
  • Strong clinical assessment and diagnostic skills
  • Excellent communication and interpersonal abilities
  • Experience in post-acute care preferred, but not required (training available)
Expected compensation is estimated salary range of $88,000 to $125,000 annually with no cap on productivity income potential. This full-time role is benefit eligible.
