Job Description
We are seeking a compassionate and dedicated Nurse Practitioner or Physician Assistant to join our innovative Mobile Care Program. This role focuses on delivering high-quality medical care to elderly patients within cluster care facilities through a fully equipped mobile unit, as well as providing direct in-home care services. You will play a critical role in improving access to healthcare for vulnerable populations, enhancing their quality of life, and providing comprehensive care in a collaborative environment.
Location: 1 in Bronx, 1 in Brooklyn, 1 in Queens.
Key Responsibilities:
- Patient Care: Provide assessments, diagnoses, and management of acute and chronic medical conditions for elderly patients.
- Mobile Outreach: Travel to designated cluster care facilities in a team mobile unit to deliver care, ensuring patient needs are met in a timely and efficient manner.
- In-Home Care: Deliver personalized medical services to patients in their homes, ensuring continuity of care and addressing unique health needs
- Health Screenings: Conduct routine health screenings, vaccinations, and wellness checks to promote preventive care and early detection of health issues.
- Care Coordination: Collaborate with the interdisciplinary teams, including primary care physicians, nurses, social workers, and facility staff, to develop and implement individualized care plans for patients.
- Patient Education: Educate patients and their families on health management, medication adherence, and lifestyle modifications to enhance overall well-being.
- Emergency Response: Respond to medical emergencies and provide appropriate care or interventions as needed.
- Community Engagement: Participate in community outreach efforts to raise awareness of the program and promote health services available to the elderly population.
Schedule: Full-time 40 hours per week, 8am – 5pm, M-F
Compensation & Benefits: $180,000 $190,000 plus full benefits package
Requirements:
- Licensed to practice as a Nurse Practitioner or Physician Assistant in NY.
- DEA Certification
- Board Certification
- Must have or willing to obtain a Medicaid Provider ID and a Medicare Provider ID in good standing.
- Minimum of one year of experience preferred working with a frail or elderly population, or the ability to receive appropriate training from the PACE organization
- Minimum of two years of clinical nursing practice in home care, care management, or a related field preferred
- Effective oral and written communication and interpersonal skills
