- Office Ancillary Staff: Front office, 2 medical assistants, 1 mid-level in the office, 2 mid-levels at the hospital, 3 vascular techs in the office
- EMR System: EPIC
- Trauma Level: III
- Credentialing Timeframe: 60-90 days
- Call: Yes, including holiday call and callbacks

**Submissions Requirements**
- Board Certified in Vascular Surgery
- BLS certification required
- Active CO license or IMLC required
- COVID vaccination required
- Disclosure of years of EPIC experience and last date of use required
- Clean malpractice and background history required
- Acknowledgment that credentialing will be without guaranteed dates required
