- Specialize in caring for elderly and medically frail patients in Grand Rapids, MI
- Oversee care in one to three local rehabilitation and nursing facilities
- Collaborate with skilled Advanced Practice Providers managing day-to-day care

**Position Requirements**
- Board Eligible/Board Certified in Geriatrics or Internal Medicine with geriatrics experience
