**Position Details:**
- Located in Stayton, OR - Salem 15, Portland 60m
- Full-time, permanent position
- Salaried Position
- Employed opportunity
- 32-36 hours of clinical patient care per week
- Attendant administrative time and task
- Expect to care for a spectrum of primary care concerns on a given day, seeing a panel of families and individuals through both chronic illnesses and acute concerns
- The provider is the anchor of an integrated clinical team, including medical assistants, behavioral health clinicians, community health workers, and others
- The successful candidate may be flexible in providing care across our clinic system, working with multiple colleagues from week to week, or may be located in a home clinic.
**Job Qualifications:**
- Active Licensed PA or NP in the state of Oregon
- Current DEA License
- 2 or more years of experience, preferably in rural community medicine
