Job Description
We are seeking an experienced Family Medicine Nurse Practitioner to join our dedicated team in providing comprehensive primary care services across four Phoenix-area locations. This full-time position focuses on delivering high-quality, patient-centered care to a diverse population in Midtown, Northwest Phoenix, Guadalupe, and Mesa. The ideal candidate is a compassionate professional with a solid background in family practice and a passion for serving the community.

Requirements:
- Graduate of an accredited Nurse Practitioner Program and certified by the American Nurses Credentialing Center or the American Academy of Nurse Practitioners.
- Must have a permanent and unrestricted license to practice as a nurse practitioner in Arizona.
- Must have current board certification in family medicine and maintain certification.
- Must have current DEA license.
- At least 3 years of experience in a family practice or comparable setting is required. Experience in a community health center setting is preferred.
- Current ACLS certification.
- Must possess a valid and unrestricted Arizona driver’s license (within 30 days of hire if applicant is from out-of-state) with no DUI/DWI or reckless driving convictions in the last five years. No more than two at-fault accidents in the last three years. Must maintain a valid Arizona driver’s license during employment. Must be insurable under our liability auto policy.
- Must pass a criminal background check and obtain a Class I Fingerprint Clearance Card within the initial ninety (90) calendar days of employment and retain a valid card while employed.
- Must pass substance abuse testing upon employment and submit to random testing during employment.
- Must provide an updated immunization (IZ) record at the time of employment.
- Must have a current (within 12 months) Tuberculosis (TB) skin report upon employment and provide an annual update.
- Must obtain a First Aid/CPR Card within (90) calendar days of employment and maintain a valid CPR Card during employment.
