Job Description

We are seeking a highly skilled and compassionate Family or Internal Medicine Physician Assistant or Nurse Practitioner for a locums position in Colebrook, NH. This outpatient clinic provides primary care to a diverse patient population, ranging from pediatric to geriatric care, with a high elderly population and multiple comorbidities. The position is Monday through Friday, 8 AM to 5 PM, with a 12-1 PM lunch break. The ideal candidate must be comfortable working autonomously, with strong experience in chronic disease management and patient education.  The position involves a caseload of 16+ patients per day and focuses heavily on chronic disease management, as well as having difficult conversations with patients regarding medication management and patient education.  The role also includes educating and counseling patients, especially regarding medication management and lifestyle changes. You will conduct thorough patient assessments, diagnostic evaluations, and create personalized treatment plans based on each patient's needs.

Work Schedule:

  • Hours: Monday – Friday, 8:00 AM to 5:00 PM, with a 12:00 PM to 1:00 PM lunch break.
  • Administrative Time: 4 hours of administrative work each week, built into the schedule.

Qualifications:

·         Must be licensed as a Physician Assistant (PA) or Nurse Practitioner (NP) in the state of New Hampshire (NH).

·         Certified by the National Commission on Certification of Physician Assistants (NCCPA).

·         Minimum of 3 years of experience in outpatient primary care, with demonstrated expertise in managing chronic diseases and comorbidities.

·         Comfortable working autonomously in a solo practice setting, utilizing strong clinical judgment and decision-making skills.

·         Excellent communication skills to effectively address difficult conversations with patients and provide patient education.

·         Ability to work in a remote area and adapt to cold weather and winter conditions.

·         Must be comfortable managing a high elderly patient population and addressing multi-comorbidity cases.
