Baptist AgeWell Center for Health, is a senior-focused service that utilizes multiple modalities to provide exemplary care for our patients in Northeast Florida. Located downtown in the One Call Building on the Baptist Medical Center of Jacksonville campus, we're seeking a passionate Physician with dedication for senior patients. We offer our physicians the opportunity to have a meaningful impact on the lives of the patients they serve without losing their way in the business of healthcare.   

 

What You Can Expect

  • Lead your care team, collaborating with experienced Advanced Practice Providers 
  • Support from an extensive in-practice team consisting of behavioral health, social work, pharmacy, nutrition, care coordination, and rehab resources .
  • Locally governed, physician led, professionally managed and community focused program ensures autonomy and a voice in your practice 
  • Epic EHR 
  • M to F 8 am to 5pm
  • On call 2 to 3 weeks per year for Monday through Sunday.

What You Will Need 

  • MD or DO 
  • Active/unrestricted medical license 
  • Board certified/eligible  
  • Family Medicine or Internal Medicine trained with a passion for senior care and population health.
