In this part-time position, you will enjoy flexibility in scheduling while providing high-quality care to older adults in skilled nursing and rehab settings. - Flexible Schedule: Enjoy a 1-2 day per week rounding schedule that allows for an ideal work-life balance. - Competitive Compensation: Expected salary range of $132,000 to $159,000 annually, with uncapped productivity-based income potential. - Unmatched Flexibility: Design your own schedule to fit your personal life and career goals. - CME Support: Access to continuing medical education to support your career development. - Part-Time Benefits: Professional liability insurance with tail coverage, wellness benefits, license reimbursement, and more. - Sign-On and Relocation Incentives: We offer relocation incentives to help with your transition. **Responsibilities** - Patient Care: Round on patients 1-2 days a week in skilled nursing and rehab facilities, providing comprehensive care (H&P, progress notes, admissions, discharges, follow-up visits). - Collaboration: Work closely with interdisciplinary teams, facility leadership, and other healthcare professionals to ensure optimal care and outcomes. - Care Transitions: Help coordinate patient transfers to appropriate care settings based on their needs upon discharge (home, independent living, assisted living, or memory care). - Clinical Leadership: Foster a culture of care excellence and provide leadership within the facility, improving patient outcomes and enhancing the quality of life for older adults. **Qualifications** - Board Certified or Board Eligible: In Family Medicine (FM) or Internal Medicine (IM). - Passion for Geriatrics: A dedication to serving older adults and improving their quality of life. - Leadership Skills: Desire to play a leadership role within a post-acute care setting. - Experience in Post-Acute Care: Preferred, but not required.
