**Locums General Dentistry - Eureka, CA**
**Coverage Need**
- Assignment Dates: ASAP - Ongoing
- Minimum 3-month commitment required
**Position Details**
- Schedule: Monday-Friday, 8 AM-5 PM
- Patient Mix: 50% Pediatrics / 50% Adults
- Coverage Type: No Call
- EMR: Epic
- Holidays Off: New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving, Christmas
- Nearest Airport: ACV (Arcata-Eureka Airport)
- Lodging: Contact HCO for assistance, may have in-house housing
**Submissions Requirements**
- Active CA license only
- Board Certified/Eligible
- Clean History
- BLS Required
**Addiitional Details**
- Active DEA needed before start
- Immunization Requirements (before start date):
- 2022-2023 Flu Vaccine
- TB Status/X-Ray (within 90 days of start)
- MMR, Hep B, Varicella Zoster Titers or Immunization Series
- Tdap (within last 10 years)
- Chart closure within 72 hours
- Credentialing Timeframe: 30-60 days (6 weeks standard for timely submissions)
