**Submissions Requirements**
- WA License or IMLC Required
- Clean Background: Highly Preferred
- Credentialing Timeframe: 90 Days
- Temporary Privileges Available: No

**Skill Requirements**
- Upper GI Endoscopy (with biopsy, foreign body removal, and fulguration of bleeder)
- Esophageal Dilatation/Manometry
- Colonoscopy (with biopsy and excision of polyp)
- Sigmoidoscopy (Flexible & Rigid)
- Small Bowel Biopsy
- Lower GI Endoscopy

**Rates**
- Regular Hourly: $350.00
- OT/Holiday: 1.5x
- Nightly Call Max: $353.50
- 24 Hour Call Max: $2500.00
- 24 Hour Call Hours Min: 3
- Call-Back Hourly: $425
