Job Description
As a Palliative Care specialist you will play a crucial role in delivering exceptional care to our patients. - Conduct comprehensive assessments of patients' health status and symptoms
- Develop and implement individualized care plans in collaboration with other healthcare providers
- Manage medications and treatments to alleviate pain and other distressing symptoms
- Provide emotional and psychological support to patients and their families
- Educate patients and families about illness progression and available treatment options
- Coordinate with social workers, chaplains, and other healthcare professionals to ensure holistic care
- Facilitate advanced care planning and end-of-life discussions
- Monitor and evaluate patient progress, adjusting care plans as needed
- Participate in interdisciplinary team meetings to review and update patient care plans
- Advocate for patients' needs and preferences within the healthcare system
- Conduct and contribute to research in palliative care practices and outcomes
- Minimum of 2 years of experience in a palliative care setting
- Master's degree in nursing, with a specialization in palliative or hospice care
- Experience in managing chronic pain and symptoms related to serious illnesses
- Experience working within a multidisciplinary healthcare team
- Familiarity with electronic health records and palliative care documentation
- Training in advanced care planning and end-of-life care discussions
- Active nurse practitioner (NP) license and DEA for Washington state
- Strong clinical assessment and diagnostic skills
- Demonstrated ability to manage complex patient cases
- Proficiency in patient education and counseling
- Commitment to continuous professional development
Our compensation is a productivity-based model (Fee for Service) that offers you control to maximize your income potential. Expected compensation is estimated salary range of $138,000 to $164,000 annually with no cap on productivity income potential. This full-time role is benefit eligible.
