Job Details

• Working just outside of Richmond 
• 5-10 patients per day
• Full-time position
• Competitive compensation, with an estimated salary range of $78,000 annually to $84,000 annually with no cap on productivity income potential

Responsibilities

• Drive high-quality, safe, and cost-effective patient care in post-acute care communities
• Collaborate with other clinician interdisciplinary teams and follow CMS guidelines
• Work closely with hospice, palliative care, and home care agencies for safe patient transitions
• Ensure timely documentation and billing through our EMR (Gehrimed) meeting Medicare guidelines

Qualifications

• Willing to train experienced candidates 
• Two years of psych experience and skilled nursing facility (SNF) experience ideal
