TeamHealth is looking for a nurse practitioner (NP) or physician assistant (PA) to work full-time (5 days per week) doing primary care rounding in a skilled nursing facility in the Syracuse, New York, area.
Key Responsibilities
Expected compensation is estimated salary range of $115,374 to $137,000 with no cap on productivity income potential.

TeamHealth is looking for a nurse practitioner (NP) or physician assistant (PA) to work full-time (5 days per week) doing primary care rounding in a skilled nursing facility in the Syracuse, New York, area.
Key Responsibilities
- Perform primary care in a post-acute setting for short term and long-term patients
- Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care
- Order and interpret lab results
- Prescribe and monitor medications
- Consult with physicians and coordinate patient care with other healthcare practitioners
- Educate patients and their caregivers on coping skills to handle chronic and age-related conditions
- Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery
- Promote healthy lifestyle changes
- A minimum of 1 year experience as a nurse practitioner in a primary care setting required
- Post-acute care experience a plus, but not required
- Active and unrestricted New York NP or PA license required
- Active and unrestricted DEA with prescriptive authority required
