Job Description

In this full-time (5 days per week) primary care role, you will have the opportunity to manage complex patient cases in a post-acute setting (SNF) in Philadelphia, PA, focusing on improving care for some of the most vulnerable patients. You will work alongside a collaborative interdisciplinary team and lead initiatives that make a tangible difference in patient health and quality of life.

Job Duties
  • Primary care rounding in skilled nursing facilities 
  • Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care
  • Collaborate with clinicians, facility leaders and others regarding care and services
  • Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery
  • Provide oversight, guidance, and mentorship to our nurse practitioners
  • Conduct clinical case reviews, as needed and share tools, information, and guidelines as they relate to quality of care
  • Represent TeamHealth on appropriate external levels engaging and establishing/maintaining relationships within the post-acute care community
  • Collaborate with leadership to review care coordination initiatives, develop collaborative intervention plans, and share ideas about network management issues

Qualifications
  • Minimum of 1 year of experience, with a commitment to delivering high-quality, patient-centered care
  • Post-acute care experience is beneficial, though not required, as we provide comprehensive support and development opportunities
  • Active and unrestricted Pennsylvania license (or in process)
  • Active and unrestricted DEA/prescriptive authority (or in process)
  • A deep commitment to improving patient outcomes, leading clinical excellence, and driving change in the healthcare landscape
  • Internal medicine, family medicine, emergency medicine, hospitalist or candidates with any related experience are welcome to apply. 

Compensation
Compensation is fee for service (fee-for-service). Expected compensation is an estimated salary range of $180,000 to $291,000 annually with no cap on productivity income potential.
