* Dates: ASAP - Ongoing
* Rate: Negotiable (tell us your rate)

**Job Details:**
* Practice Setting: Inpatient
* Coverage Type: Scheduled Clinical Hours + Call
* EMR System: Cerner
* Patients Per Shift: 5
* Call Type: Beeper
* Call Ratio: 1:5
* Call Response Time: 30 minutes
**Requirements:**
* Board Certified CRNA (NBCRNA): Required
* Active AZ Medical License: Required
* ACLS & BLS (AHA or American Red Cross): Required
* Must be comfortable practicing independently and performing peripheral nerve blocks
* Case logs of blocks performed: Required
* Must be willing to be credentialed and work at multiple locations
* Must be willing to work nights, weekends, and take call
* Provider cannot be within 60 miles of the facility
**Support Staff:**
* RNs: 20
* PA/NPs: 5
