- Licensed in CA only
- Active DEA before start
- BLS certification required
- CV must include months & dates for work experience with all gaps accounted for
- Immunization requirements:
- 2022-2023 Flu Vaccine
- TB Status/X-Ray Results (within 90 days of start)
- Positive MMR, Hep B, Varicella Zoster Titer or complete immunization series
- Tdap (within last 10 years)
- COVID-19 Vaccine no longer required
- Credentialing: 30-90 days

**Skill Requirements**
