Remote Advanced Practice Clinician Leader

Essential Duties and Responsibilities
Medical Record Review and Analysis

• Conduct thorough reviews of patient medical records to identify gaps in care, potential risks, and areas for improvement
• Evaluate treatment plans for alignment with best practices and evidence-based guidelines
• Document findings and actionable recommendations clearly and concisely

Improvement Plan Development

• Develop and implement quality improvement initiatives to enhance patient care and operational processes using StayPut data
• Collaborate with regional and market teams to identify and address areas for improvement
• Focus on chronic condition management within patients' current care environments

Clinical Decision-Making Support

• Provide expert guidance to caregivers and staff to prevent avoidable hospital admissions and ED visits
• Address barriers to effective care delivery through targeted interventions

Education and Training

• Offer clinical education and quality improvement coaching to clinicians in the Call Solution team
• Educate nursing and care staff on best practices and early intervention techniques
• Provide ongoing onboarding and education for clinical team members

Interdisciplinary Collaboration

• Work closely with physicians, nurses, social workers, and other team members to ensure comprehensive patient care
• Participate in interdisciplinary team meetings to review patient progress, care strategies, and program goals

Program Development and Evaluation

• Contribute to refining StayPut protocols and workflows to enhance patient outcomes
• Develop and maintain StayPut resources to promote the program within partner facilities
• Analyze program metrics to evaluate effectiveness and identify areas for improvement

Compliance and Quality Assurance

• Ensure adherence to regulatory and clinical standards
• Participate in audits and quality assurance initiatives as needed

 
Qualifications and Experience

• Education: Master's Degree in nursing or Master's Degree in physician assistant studies
• Licensure: Active nurse practitioner (NP) or physician assistant (PA) license
• Experience: Minimum 3 years' experience as NP or PA in post-acute care or quality improvement initiatives

• Strong knowledge of quality improvement methodologies and tools

• Strong analytical and problem-solving abilities
• Proficiency in data analysis and reporting
