About the Role
You will play a vital role in delivering compassionate and effective care to our patients in post-acute care communities. Your responsibilities include:
- Providing palliative services to a panel of 15-20 patients per day
- Collaborating with interdisciplinary teams to optimize patient outcomes
- Ensuring smooth transitions of care
- Maintaining accurate and timely documentation in our EMR system (Gehrimed)
- Work-life balance: enjoy a flexible Monday through Friday schedule

- Industry recognition: be part of a nationally recognized healthcare leader
- Graduate degree as a nurse practitioner or physician assistant
- Active and unrestricted NP or PA license in New York
- Strong clinical assessment and diagnostic skills

- Experience in post-acute care preferred, but not required (training available)
