• Must have active state license
• BC required
• Weekdays, Days
• BLS required
• 20 patients per day, must be able to see all ages but will consider peds or adult only providers
• Emphasis on well child visits, women's annual physicals, Medicare wellness visits, walk-in visits
• Credentialing needed
• DEA needed
