**Coverage Need**
- Assignment Dates: Starting October 13, ongoing 3-6 months
- Specific dates: 10/13-17, 10/20-25, 10/27-31, 11/3-7, 11/17-21, 11/24-28
- Additional coverage starting week of December 15
- Minimum commitment: 3 months
- Minimum shifts: 5-6 days of call plus clinic per month
- Clinic: Monday Friday, 8a 4p
- Call: 24 hours (7a 7a), night call from 5p 7a

**Submissions Requirements**
- Board Certified or Board Eligible
- Licensed in WA or IMLC (LOQ in hand)
- BLS required
- DEA required
- Must be available for listed dates or specify black-out dates
- Must indicate number of shifts available per month
- Must submit full presentation packet including CV, malpractice info, license/certifications, and background disclosures
