Position Overview:
Provide medical care to patients as part of a healthcare team, focusing on internal medicine or family practice. Support the overall mission and patient care goals of the health center.

Key Responsibilities:
Examine, diagnose, and treat patients of all ages.
Manage chronic and acute conditions, including HIV.
Refer patients to specialists as needed.
Maintain accurate and timely medical records.
Participate in on-call phone coverage on a rotating basis.
Work closely with other healthcare staff and providers.
Support and supervise clinical staff when needed.
Follow all safety, infection control, and clinical protocols.
Attend required meetings, training, and quality improvement activities.
Stay up to date with medical education and licensure.
Perform other duties as assigned.
Qualifications:
1 2 years of experience in outpatient or community health.
Certified Family Nurse Practitioner or Physician Assistant.
Licensed in New York State.
Board certified.
