**APP-Palliative Care Opening - Findlay, OH**
**Position Overview**
A growing community-focused healthcare system in Northwest Ohio is seeking a dedicated Advanced Practice Provider (APP) with experience in acute care, hospice, or palliative medicine to join a collaborative and compassionate team. This position offers a rewarding mix of inpatient and outpatient care with a strong emphasis on quality of life, chronic disease management, and patient-centered symptom support.
**Role Highlights**
Primary location at a regional acute care hospital
75% inpatient, 25% outpatient setting
Rounding on hospice patients and managing complex symptoms
5-day workweek, Monday Friday, 8:00 AM 5:00 PM
Average of 6 patients per day
Rotating on-call schedule
Hospitalists manage admissions; open ICU setting
Clinical Focus Areas
Symptom and pain management
Chronic disease support
Goals of care and advance care planning
End-of-life care coordination
Patient and family counseling
**Qualifications**
Experience in palliative care, hospice, geriatrics, or acute care preferred
Ability to practice autonomously in a supportive, team-based setting
Strong communication and interpersonal skills
Eligible for medical licensure and credentialing in Ohio
**Compensation & Benefits**
Competitive base salary with quality bonus potential
200 hours of vacation and CME time
